Abstract:

Aim — to compare long-term results of bilateral internal mammary artery (BIMA) grafting by using of in situ and free graft techniques. Material and methods. Function of 108 mammary conduits were analyzed: 54 free and 54 in situ grafts. Mean time from surgery till control angiography was 16±5 months. Results. Satisfactory function of in situ internal mammary artery (IMA) was detected in 94.4% of cases and in 90.6% of cases for free grafts. Outcomes of BIMA grafting with different types of conduits depended on region of revascularization. In situ IMA was significantly better for anterior and anterolateral left ventricular walls compared with free graft (97.5 and 100% vs. 93.3 and 66.7%). In case of posterolateral left ventricular wall revascularization free IMA graft is preferable over in situ conduit (92.9 and 75%, respectively). Normal function of free IMA graft was noted in 100% after moderate coronary stenosis bypass (60—70%) and only in 77.8% for in situ conduits (p<0.05). Conclusion. In BIMA grafting of circumflex artery branches and moderate coronary stenoses (60—70%) free IMA grafts should be preferred over in situ conduits.
