Aim - to analyze our own results of aortic stenting for dissection types A and B within various terms. Material and methods. The study included 42 patients with aortic dissection types A and B who underwent thoracic aortic stenting. This method was used alone as well as in hybrid approach. Immediate success of intervention and in-hospital major cardiovascular complications were evaluated. In long-term period we assessed survival, major cardiovascular events and development of new boundary fenestration at the level of distal stent graft. Results. Technical success of replacement was achieved in all patients. Mortality was not observed. Acute cerebral vascular accident (CVA) occurred in 1 (2.4%) patient, significant increase of cardiac enzymes - in 1 (2.4%) patient, retrograde dissection - in 2 (4.7%) cases, endoleaks I or III type - in 1 (2.4%) case. Long-term survival rate was 88.1%. 2 (4.7%) patients had stroke. New boundary fenestration was revealed in 6 patients (17.6% of patients with CT data and 14.2% of the total). Conclusion. The development of new boundary fenestration at the level of distal stent graft is frequent event after aortic stenting for dissection. Algorithm of distal graft sizing in relation to true lumen of descending aorta is currently absent. This problem may be theoretically resolved by new devices taking into account the features of clinical course and evolutionary changes of dissected aorta.
