Introduction of miniinvasive surgeries (laparoscopic, thoracoscopic, endoscopic) to broad surgical practice as well as ongoing development of endoscopic instrumentation gave the possibility to significantly reduce negative outcomes of the surgical procedure, surgical trauma, postoperative pain syndrome, rate of complications, time of rehabilitation and hospital costs. However opinions of trocar placement to create better conditions for operation are still kept open. Every surgeon is guided by one's own experience and concepts of "comfort" of the surgical procedure. We attempted to find out optimal points for thoracoports insertion in accordance with certain criteria of surgical approach which include axis of operation, depth of the wound, angle of operation, area of availability, endosurgical angle of operation and "working pyramid" (these criteria can be estimated by different measurement means). Results obtained may be successfully applied in treatment of patients and for optimizing surgical approaches. We accomplished topographo-anatomic research due to which we ascertained optimal trocar placement in three-port videothoracoscopic upper lobe pulmonary resection. Effective trocar placement is: 3rd and 5th intercostal space along the anterior axillary line and 6th intercostal space along the posterior axillary line.
