The results analysis of diagnosis and treatment of 109 patients with generalized purulent peritonitis has been carried out. Patients were divided into two statistically homogenous groups. Group 1 consisted of 68 patients after conventional two-stage operations which included elimination of source of peritonitis at the 1st stage and abdominal cavity videoendoscopic sanation at the 2nd. 41 patients of group 2 underwent the same operations except that second-stage peritoneal defibrination was performed by pulse jet of antiseptic and abdominal cavity was sanitated with 200 ml immobilized form of sodium hypochlorite in carboxymetylcellulose gel instead of aqueous solution. In the immediate postoperative period indicators of endotoxicosis were 1,5 times lower and motor-evacuation function of the intestine was 2 times higher than in 1st group patients, that resulted in reduction of postoperative complications by 9% and postoperative mortality - by 4%. Indications and contraindications to the techniques given are defined on the basis of abdominal sepsis severity, intraabdominal hypertension, changes of APACHE-II score and Manheim peritonitis index.
