
Мазь такролимуса: селективный T-клеточный иммуносупрессант в дерматологии

Мазь такролимуса — топический ингибитор кальциневрина с выраженным противовоспалительным действием, подавляющий активацию T-лимфоцитов и продукцию провоспалительных цитокинов. В отличие от топических глюкокортикостероидов, препарат не вызывает атрофии кожи и истончения эпидермиса, что делает его особенно ценным для длительного применения в стероидчувствительных зонах — на лице, в кожных складках и аногенитальной области. Помимо основного противовоспалительного эффекта, мазь такролимуса обладает клинически значимым противозудным свойством и фунгистатической активностью в отношении Malassezia spp., что расширяет спектр ее терапевтических возможностей. Единственным официально зарегистрированным показанием остается атопический дерматит, однако за последние 20 лет независимыми исследователями накоплена обширная доказательная база, охватывающая более 50 нозологий, — от витилиго, красного плоского лишая и кожных форм красной волчанки до склероатрофического лихена, розацеа, себорейного дерматита и псориаза деликатных локализаций. По ряду из них доказательная база включает рандомизированные контролируемые исследования и метаанализы. Препарат вошел в отечественные и международные клинические рекомендации более чем по 20 нозологическим единицам. Настоящая публикация посвящена систематическому анализу фармакодинамического профиля мази такролимуса и обоснованию ее применения за пределами атопического дерматита. В качестве иллюстрации представлены 6 клинических случаев из практики врачей-дерматовенерологов разных регионов России: дискоидная красная волчанка, розацеа, себорейный дерматит, аллергический контактный дерматит лица, себорейный псориаз и узловатое пруриго. Во всех случаях применение мази такролимуса позволило достичь выраженного клинического улучшения при хорошей переносимости, в том числе у пациентов с предшествующей стероидной зависимостью или в анатомических зонах, где длительное использование топических глюкокортикостероидов нежелательно.
