OUTCOME OF MULTIFETAL PREGNANCY (A REVIEW)

THE THERAPEUTIC EFFECT OF DESOGESTREL IN WOMEN WITH MASTALGIA AND MASTOPATHY

INDOLECARBINOL EFFICACY FOR MASTALGIA TREATMENT

The efficacy indolecarbinol for mastalgia treatment was studied in randomized double-blind placebo-controlled multicenter clinical trial. 62 patients received 400 mg/day of indolecarbinol or placebo for 6 menstrual cycles. The success rate in therapy group was significantly higher than in placebo group (83.3% vs. 50.3%, p=0.002). Plasma concentration of sex steroid-binding globulin increased and free estradiol level decreased in Indinol Forto group. Increased ratio of urinary estrogen metabolites (2-hydroxyestrone/16α- hydroxyestrone) was observed in indocarbinol group. Indocarbinol is recommended for the treatment of patients with both cyclical mastalgia and mammary dysplasia.

THE PRESERVATION OF REPRODUCTIVE FUNCTION IN PATIENTS WITH INOPERABLE CERVICAL CANCER

In this paper two cases of ovarian stimulation in patients with inoperable cervical cancer are presented. The use of surrogacy may be helpful for these patients. One successful case of such approach is described. By our knowledge, this is the first published case of pregnancy after chemotherapy in patient with cervical cancer stage IIb.

THE ROLE OF ANGIOGENESIS MARKERS IN ASSISTED REPRODUCTIVE TECHNOLOGY PROGRAMS IN PATIENTS WITH DIFFERENT FORMS OF INFERTILITY

The paper presents the study of VEGF and sPECAM-1 levels in blood serum and follicular fluid in patients with different forms of infertility. We revealed the negative effect of high level of VEGF in blood serum on embryo implantation in tubal-peritoneal infertility and reverse interaction in endocrine infertility. The high concentration of VEGF in blood serum induces the increase of sPECAM-1 concentration in follicular fluid in induced cycles.

VITRIFICATION OF OVARIAN TISSUE AS A METHOD OF EARLY STAGE OOCYTES SAVING

The efficiency of ovarian tissue vitrification in preserving follicular reserve was investigated. Thawed ovarian tissue showed good morphological parameters and tissue viability. After orthotopic transplantation of thawed ovarian tissue in the peritoneum of the anterior abdominal wall of the laboratory animals the follicle growth in response to gonadotropin stimulation was demonstrated.

EFFECTIVENESS OF ARRAY-COMPARATIVE GENOMIC HYBRIDIZATION APPLICATION FOR PREIMPLANTATION GENETIC TESTING OF EMBRYOS IN PATIENTS WITH MULTIPLE IVF FAILURES

The effectiveness of array-CGH for preimplantation screening of embryos in patients with multiple IVF failures was studied. It was shown that the selection of embryos by genetic screening allows to reach the pregnancy rate of 63.3% per transfer versus 13.3% in control group. The preimplantation genetic screening in patients with multiple IVF failures was proved to increase the efficiency of ART.

THE DIAGNOSTIC VALUE OF NEOPTERIN LEVELS IN BIOLOGICAL FLUIDS OF PATIENTS WITH PROSTATE PATHOLOGY

The neopterin concentration in seminal plasma of patients with chronic prostatitis was investigated. Sperm samples from 50 patients were studied. Neopterin was revealed in 60% of microorganisms, 83.3% of them were staphylococcus, mainly Staphylococcus epidermidis (concentration in cultivated fluid was 4,54±0,21 nmol/l). In Staphylococcus aureus culture neopterin was found in 66.6% of strains at concentration of 1,5±0,08 nmol/l. 44,4% of Escherichia coli cultures were neopterin positive with mean concentration of 2,543±0,12 nmol/ml. Biochemical aspects of neopterin functioning as antibacterial factor were discussed.

DINITROSYL IRON COMPLEXES WITH GLUTATHIONE REDUCES THE PAIN IN RATS WITH EXPERIMENTAL ENDOMETRIOSIS

The effect of binuclear dinitrosyl iron complexes (DNIC) with glutathione on endometrioid tumors and painful manifestations in rats with experimental endometriosis have been studied. Two fragments of endometrium with myometrium (2×2 mm) from the left uterine horn was grafted to the inner surface of the anterior abdominal wall. The test animals received intraperitoneal injections of 0.5 ml DNIC-glutathione at the dose of 12.5 micromole per kg daily for 12 days. The injections resulted in more than a 2-fold decrease in the total volume of both large tumors formed from grafts. Painful manifestations in the experimental group significantly decreased as compared with the control group. Breach of transferrin structure was recorded in animals of the experimental and control groups. The degradation resulted in release of a high amount of nitric oxide molecules and nitrosonium ions from DNICs affecting the tumors by the way of the cytotoxic effect.

ESTROGEN METABOLITES IN PATIENTS WITH EXTERNAL GENITAL ENDOMETRIOSIS

The content of estrogen metabolites in patents with external genital endometriosis was studied. Proliferative metabolites (16α-hydroxyestrone 16α-OHE1) level in women with severe external endometriosis was significantly higher than the anti- proliferative metabolites (2-hydroxyestrone 2-OHE1) level. After combined surgical and medical treatment the pregnancy rate in patients with predominance of anti-proliferative or proliferative metabolites was 73.5% and 53.5%, respectively. Possible balance between estrogen metabolites in the urine may be recommended as a predictor of treatment efficacy of external genital endometriosis.

MODERN ASPECTS OF PREGNANCY PLANNING IN WOMEN WITH DIABETES

Pregnancy is a complex metabolic state, which includes dramatic changes in hormone secretion and metabolism. Women with diagnosed diabetes always are of greater risk of obstetric complications, which can be prevented by eliminating risk factors, early diagnosis and careful management of pregnancy and postpartum period.

OBSTETRIC MANAGEMENT AND OUTCOME OF MULTIFETAL PREGNANCY: FETUS SELECTION FOR REDUCTION

The reduction of fetus from monochorionic fetuses in dichorionic triamniotic triplet or trichorionic tetraamniotic quadruplet pregnancies was successfully made. The fetus death rate of second monochorionic fetuses was 75% at 3 days or less after procedure. Multifetal pregnancy reduction is an efficient and safe way of improving outcome in high — order pregnancy.
