Mitotic activity in glandular epithelium and stromal cells of ectopic endometrium at patients with peritoneal form of endometriosis is associated with an expression of apoptosis activators and inhibitors. These data demonstrate a pathogenetic role of proliferative activity and apoptosis in ectopic endometrium in patients with the active form of endometriosis.

THE TREATMENT OF WOMEN WITH ANDROGENIC DERMATOPATHY

The treatment of women with androgen dermatopathy remains the most difficult problem. The complex approach to the treatment is necessary in accordance with the origin of hyperandrogenia. The hormonal, metabolic and oxidative status of the patients with clinical signs of the hyperandrogenism of the various genesis (PCOS, CAH, the idiopathic form) is discussed. It has been shown that antioxidative therapy of androgen dermatopathy may intensify treatment efficiency.

DIAGNOSIS AND TREATMENT OF CHRONIC ENDOMETRITIS IN WOMEN WITH PREVIOUS MISCARRIAGES

159 women with previous miscarriages were investigated and in 105 of them (66%) chronic endometritis was revealed. The immunohistochemical method with monoclonal antibodies to NK cells (CD16+, CD56+), HLA-DR+ and B-lymphocytes (CD20+) was used for diagnosis of chronic endometritis. More than a half of patients had chronic endometritis with autoimmune component (AIC) leading to the increase of antiendometrial antibodies (AEA) concentration in peripheral blood. The patients with chronic endometritis and AIC had significantly delayed secretory transformation of endometrium despite of normal ovarian luteal activity. AEA could have independent influence on hormone-receptor interaction provoking thus morphologic and functional disorders of endometrium. Complex therapy of chronic endometritis led to elimination of AEA and normalization of secretory transformation of endometrium.

LAPAROSCOPIC HYSTERECTOMY: EFFECT ON SEXUAL FUNCTION

The frequency of sexual dysfunction after hysterectomy varies from 10 to 40%. Effect of laparoscopic hysterectomy on sexual function in women associated with hormonal disorders (reduced libido, arousal, vaginal dryness) that may be associated with damaged vascularisation of the ovaries after surgery. Orgasm disorders rarely complicated hysterectomy. It is important to discuss with the patients the issues related to sexual function after surgery to avoid common fears.

THE IMPACT OF THROMBOPHILIA ON IVF FAILURE

The aim of the study was to investigate the incidence of thrombophilia in women with IVF failure and to develop the algorithm of patients preparing to next IVF attempt. 81 patients were examined. Genetic forms of thrombophilia were revealed in 75,3%. The use of anticoagulant therapy in IVF programs resulted in high pregnancy rate (50.0%).

Submucosal and intramural fibroids that distort the endometrial cavity have been associated with decreased pregnancy rates following IVF treatment. IN VITRO FERTILIZATION AND MANAGEMENT OF HAEMOSTASIS DISORDERS

The association of excess thrombin production and hypofibrinolysis is associated with IVF failures, reducing the chance of pregnancy in 12.1 times. On the basis of selected laboratory parameters three groups of women were formed for therapeutic correction (heparin prophylaxis and/or calf-thigh compression) in the cycles of IVF. The treatment of 98 women with hemostasis disorders resulted in pregnancy rate increase in 4.7 times.

Early check-ups during pregnancy, self-test the pH-balance of the vaginal environment, sanitation and subsequent recovery of normal vaginal flora, the use of preventive antithrombotic therapy in cases of diagnosed hereditary thrombophilia, the use of the Actim Partus test and the adequate tocolytic therapy reduce the incidence of preterm birth and get a higher percentage of live full-term infants.
