On the use of assisted reproductive technologies, contraindications and restrictions on their use
In accordance with paragraph 11 of part 2 of Article 14, part 8 of Article 20, paragraph 2 of part 1 of Article 37 and part 2 of Article 55 of the Federal Law of November 21, 2011 No. 323-FL "On the basis of health protection of citizens in the Russian Federation" (Collected Legislation of the Russian Federation, 2011, No. 48, Art. 6724; 2019, No. 52, Art. 7770) I order:
1. To approve:
the procedure for the use of assisted reproductive technologies, contraindications and restrictions to their use according to Annex No. 1;
the list of contraindications for the use of assisted reproductive technology and artificial insemination in accordance with Appendix No. 2;
an insert to the medical record of a patient receiving outpatient care (Form No. 025/u) or an inpatient card or a card of an obstetrician-gynecological patient under the conditions of day care when using assisted reproductive technologies and artificial insemination according to Appendix No. 3;
the form of the individual card of the sperm donor according to Appendix No. 4;
the individual oocyte donor card form according to Appendix No. 5;
the form of the logbook of accounting, storage and use of cryopreserved sperm of patients according to Appendix No. 6;
the form of the logbook of accounting, storage and use of cryopreserved donor sperm in accordance with Appendix No. 7;
the form of the logbook of accounting, storage and use of the patients' cryopreserved oocytes in accordance with Appendix No. 8;
the form of the logbook of accounting, storage and use of cryopreserved donor oocytes according to Appendix No. 9;
the form of the logbook of accounting, storage and use of patients' cryopreserved embryos according to Appendix No. 10;
the form of the logbook of accounting, storage and use of cryopreserved donor embryos according to Appendix No. 11;
the form of the logbook of accounting of artificial inseminations according to Appendix No. 12;
the form of informed voluntary consent to medical intervention with the use of assisted reproductive technologies and artificial insemination in accordance with Appendix No. 13;
The informed voluntary consent form for medical intervention by means of embryo(s)/fetal reduction surgery according to Appendix No. 14.
2. This order shall take effect on January 1, 2021.
