Combined oral contraceptives (COCs) are a reliable method of preventing unwanted pregnancy with many important non-contraceptive properties that have been successfully used to treat gynecological diseases. The safety profile of drugs has particular importance with long-term use, especially when a woman continues to use hormonal contraception until menopause. The article reviews the literature on the safety of COCs during long-term use up to menopause, according to domestic literature and information in electronic international databases PubMed, Web of Science, Cochrane Library and Scopus. It has been shown that numerous randomized trials, as well as real-world evidence data, confirm the possibility of long-term use of COCs from the beginning of the reproductive period to menopause, considering indications and contraindications. The article considers the possibility of long-term use of the estradiol valerate and dienogest combination as one of the most studied in various aspects of gynecological practice. It has been demonstrated that long-term use of combined hormonal contraception, considering indications and contraindications, has a positive balance of benefits and risks. Women who use reliable contraception are not only protected from unwanted pregnancies and subsequent abortions, but also benefit from numerous research-proven curative and preventive non-contraceptive benefits.
