OBJECTIVE
To develop a method of oncoplastic reduction mammoplasty without formation of a pedicle of the nipple-areolar complex.
MATERIAL AND METHODS
To evaluate the effectiveness of this method, we analyzed surgical treatment of breast cancer in 37 patients. All patients were divided into two groups: 22 women in the main group and 15 ones in the control group. All patients underwent bilateral reduction mammoplasty (oncoplastic procedure with mandatory regional lymph node dissection on the affected side and symmetrizing procedure without lymph node dissection on the intact side). Technique of reduction mammoplasty was identical on both sides. In the main group, patients underwent oncoplastic reduction mammoplasty without formation of a pedicle of the nipple-areolar complex. The nipple was formed from the skin of the breast using 1 out of 3 proposed methods. In the control group, women underwent oncoplastic reduction mammoplasty with variants of a pedicle of the nipple-areolar complex.
RESULTS
Analysis of data showed the advantage of oncoplastic reduction mammoplasty without formation of a pedicle of the nipple-areolar complex and formation of the nipple from the skin of the breast. This method is simpler, reduces surgery time, increases resection quality due to removal of nipple-areolar complex and prevents complications associated with pedicle of the nipple-areolar complex. This procedure also ensures favorable aesthetic and functional results.
CONCLUSION
Original method ensures total mastectomy, favorable symmetric, functional and stable aesthetic result. Moreover, we obtain natural nipple-areolar complex without periareolar scar.
