Improving the results of soft tissue lipofilling is an urgent objective of modern plastic surgery. All three surgical stages should be considered to improve engraftment of adipose autografts (fat harvesting, fat processing before injection into soft tissues and lipofilling per se). Adipose tissue transplantation and engraftment depends on the biological properties of lipoaspirate. Liposuction as the first stage of lipofilling is primarily considered in this manuscript. The following factors influencing the properties of primary lipoaspirate were determined: optimal donor area, liposuction technique, diameter and properties of liposuction cannula, negative pressure magnitude in the vacuum pump, injection of anesthetics before fat harvesting.
