Aim — to present current aspects of the etiology, pathogenesis, diagnosis and treatment of eosinophilic esophagitis (EoE). Main statements. Study of EoE has reached qualitatively new level in recent years: large randomized controlled trials and systematic reviews have been performed and published, key points of EoE etiopathogenesis have been revised. Today, the relationship between EoE and specific antigens is being questioned, whereas genetically determined impairment of barrier function of the esophagus and susceptibility to immune response through the T-helper cells type 2 activation are recognized as key factors in pathogenesis of the disease. Differential diagnosis of EoE with gastroesophageal reflux disease is not advisable because this pathology additionally aggravates course of eosinophilic esophagitis while high doses of proton pump inhibitors is the main aspect of EoE treatment. The concept of esophageal eosinophilia resolved on the background of proton pump inhibitors administration is recognized as erroneous. Conclusion. Eoeinophilic esophagitis is a chronic, immune-mediated, inflammatory disease of the esophagus. Diagnosis of EoE is based on characteristic clinical picture of esophageal dysfunction (dysphagia) combined with histological data of eosinophilic infiltration of esophageal mucosa. Basic therapy of eosinophilic esophagitis includes dietary restrictions, proton pump inhibitors and topical corticosteroids.
